Veterinary
Physiotherapy Referral

To ensure safe and legal treatment, all physiotherapy appointments require a veterinary referral/consent. We have two separate forms to manage the different referral pathways. Please read the options below to determine which form you need to complete.

Option A: Owner initiates referral (Vet referral request form)

This form should be completed by the animal’s owner or keeper.

The purpose of this form is to provide the initial animal and owner details and obtain veterinary consent.

Note for owners: After you complete this form, our system will automatically email the Vet a unique link asking them to complete the vet consent and clinical information sections.

Option B: Vet initiates referral (Direct vet referral form)

This pathway is used when a Vet wishes to directly refer a patient to us, and the owner has not yet initiated the process.

This form should be completed by Veterinary Surgeons (or authorised clinic staff).

The purpose of this form is to provide the client’s details, formal veterinary consent, share clinical history, and start the referral process.

Note for Vets: Upon submission, our system will automatically email the client (owner) a unique link asking them to complete the final ‘Owner Consent’ section.

How to refer a patient (for vets):

To refer a patient, simply download one of the veterinary consent forms above, complete the client details and clinical history sections, and return the form via email to info@cyan-mallard-268915.hostingersite.com. We will then contact the client to arrange the session, and email the clinical findings and assessment report to your clinic once the session has occurred.

Small animal (dogs/cats/rabbits) assessment and treatment – £55
Exotic / Zoo assessment and treatment – See Zoo consultancy page

Group rates available for:

Multiple dogs/cats/rabbits in a single home/setting – £45 per animal
Zoo day rate – Zoo consultancy page

Areas covered and travel expenses:

Exotics and Zoo: worldwide via online consultancy. Zoo cases seen in person throughout the UK
Horses and small animals: Cheshire, North Wales and Merseyside. No charge for travel under 15 miles from Chester. Over 15 miles, travel is charged at 45p per mile.

The General
Process Explained

Initiation: The process starts either with the Owner using Option A, or the Vet using Option B.
Consent gathering: We must receive a completed referral form, signed by the referring veterinarian.
Owner agreement: The pet owner must sign the final consent section of the form (either directly or via the automated link).
Booking: Once all necessary information and consent are secured, a member of the Shackleton Animal Physiotherapy team will contact the owner to schedule the initial consultation.
